Members accrue personal/vacation time at a rate of 13.33 hours per
month to a maximum of 20 personal/vacation days each year. Members
are eligible to take personal/vacation time as long as they have
accrued more than the number of hours they are requesting. The
company observes ten Federal holidays and provides one paid holiday
for your birthday for a total of 11 paid holidays. Members accrue
4.0 hours per month accumulated to a maximum of 48 hours per
calendar year. |

Medical
Insurance (HMO and PPO)
Medical “Opt
Out” Payments |
Medical coverage for members and their eligible dependents is
offered through Harvard Pilgrim Health Care. The HMO is available
to members residing in the New England region. The PPO is available
to all other members. Prescription drug coverage and an annual eye
exam are available under both plans. Windmill funds 85% of the
premiums. Both plans have a deductible of $500/individual or
$1500/family. As an alternative to the company sponsored medical
plan, members may elect to “opt out” of the company insurance and
provide their own health care coverage through another source such
as military retirement, individual insurance, or spouse’s medical
plan. If a member opts out, and if there is an associated cost, the
member will receive 85% of that cost as long as the costs do not
exceed the current Harvard Pilgrim Health Care premiums. |

Health
Reimbursement Account (HRA) |
Reimbursement of medical deductibles is offered to members when the
deductible is required for certain services. Members must submit
for reimbursement through our third party administrator, Group
Dynamic, Inc. |

Dental
Insurance (with adult orthodontia) |
Dental coverage is offered to members and their eligible dependents
through Guardian Life Insurance. Currently, Windmill funds 85% of
the premiums for the Guardian DentalGuard Preferred Plan. The plan
has a deductible of $50/individual and $150/family. Windmill does
not reimburse members who use another insurance carrier. |

International Medical and Security Insurance |
For our members who perform overseas business travel, additional
insurance provides extra protection. |

Long Term Care
(LTC) Insurance |
Windmill offers a voluntary Long Term Care Insurance product through
Unum Provident for yourself, your spouse, your parents, your
grandparents, spouse’s parents or grandparents, siblings, and
children over 18 years of age. The base plan consists of: benefit
duration of three years, a facility monthly amount of $2,000, and a
lifetime maximum amount of $72,000. |

Basic Life
Insurance |
Windmill pays the full cost of basic life insurance coverage for
member’s equivalent to 100% of your annual salary to a maximum of
$150,000 (if you meet eligibility requirements). |

Supplemental
Life Insurance |
Windmill offers a Supplemental Life Insurance program through
Guardian Life Insurance. Members pay the full cost of supplemental
coverage through after-tax payroll deductions, for life insurance of
up to three times your Base Annual Earnings (BAE), to a maximum of
$250,000. Supplemental life insurance coverage for a spouse or a
dependent child(ren) is also available. |

Short Term
Disability (STD) Insurance |
Windmill pays the full cost of Short Term Disability Insurance
coverage through Guardian Life Insurance. The plan covers 66 2/3%
of your current weekly salary if you suffer from a disabling
accident or illness that is not work related. You will receive
benefit payment for up to 13 weeks while you are disabled, with a
maximum weekly benefit of $1,000. |

Long Term
Disability (LTD) Insurance |
Windmill pays the full cost of Long Term Disability Insurance
coverage through Guardian Life Insurance. The plan covers 66 2/3%
of your current monthly salary if you suffer from a disabling
accident or illness on or off the job; with a minimum monthly
benefit of $50 and a maximum monthly benefit is $10,000. Benefit
payments begin after you have been unable to work for 90 days
(elimination period) due to disability. |

Flexible
Spending Accounts (FSAs) for Medical, Dependent Care, and Commuter
Choice |
Members may participate in Flexible Spending Accounts (FSAs),
through Benefit Strategies, in order to set aside pre-tax dollars
from their paycheck to pay for eligible medical, dental, and
childcare expenses not covered by other insurance. The Commuter
Choice FSA allows members to pay for eligible transportation
benefits such as a mass transit pass, qualified parking expenses,
and qualified highway vehicle expenses. |

Employee Stock
Ownership Plan (ESOP) |
Windmill is an employee owned company. Currently, 20% of the
company was sold to the ESOP trust and distributed amongst the
Windmill members. Any disbursements, when they do occur, are
subject to eligibility requirements and based upon salary and a
five-year vesting schedule. |

Simplified
Employee Pension – Individual Retirement Account (SEP-IRA) |
Windmill contributes an amount equal to 10% of your pay directly
into your personal Simplified Employee Pension – Individual
Retirement Account (SEP-IRA) through Vanguard. Members are enrolled
in Windmill’s (SEP-IRA) plan upon joining and your money is
immediately 100% vested in your investment account. |

Assistance Program |
Members are eligible to receive professional support for yourself
and your household members (not based on “insurance eligibility”).
The confidential program is intended to be used as a resource for
resolution of personal and work-related issues (family concerns,
depression, anxiety, parenting, addictions, etc.) |

Tuition
Reimbursement |
Members are eligible to receive pre-approved tuition reimbursement
after successful completion of coursework at 100% of costs up to a
maximum of $5,250 per calendar year. |

Professional
Memberships |
Members are eligible to receive reimbursement for professional
membership dues up to a maximum of $150 per calendar year.
